Participation and Consent
There is no obligation, even at this point or any point during the completion process for you to stay a part of this study. There are no consequences should you choose to not participate in the study.
There is no separate consent form for you to complete or sign. Simply completing the survey and submitting it on-line gives us your consent to use your data as part of our research analyses.
Privacy of Data
All data will be de-identified. What this means to you is that after you submit your questionnaire, we will have no way of connecting your individual responses with an email address, if indeed you have given us one. In addition, there will be no questions on the survey that will allow us to identify you from your responses in any way.
